What to expect first visit

What to expect first visit

  1. Please arrive 15 minutes early to complete paperwork. If you would like to expedite this process, please see the links to forms below, print the appropriate forms, and bring them completed to your first visit.
  2. Please bring a photo identification and insurance cards
  3. Our therapists will begin by asking you a series of questions to help better understand your impairments and get to the root of the problem
  4. The appointment will then move into a series of movement-based tests and measures to further our understanding of the issue and to develop interventions to help return you to your desired level of function and accomplish your goals.
  5. Your therapist will perform a thorough examination and discuss their findings with you. You and your therapist will work together to set goals to get you back to doing what you love. You will then be instructed in exercises and interventions to achieve those goals.


Time Frame: Initial evaluations typically last 45 minutes to one hour, occasionally they will last longer depending on the complexity of your case.
How to Dress: Please wear clothes that are comfortable and easy to move around in.

Insurance and Billing

We are a Pay at Time of Service Clinic

We will be happy to submit your claim on your behalf; however, payment is expected at time of service whether that may be co-pay, co-insurance, or payments towards deductible.

Patient Responsibilities

  • It is the patient’s responsibility to ensure that if their insurance requires a referral for treatment that a referral is provided to the practice before the first visit. Visits may be rescheduled or the patient may be financially responsible due to lack of referral.
  • It is the patient’s responsibility to provide Peak Performance PT with current insurance information, to bring their insurance card, and to notify the office of any changes in insurance.
  • It is the patient’s responsibility to check with their insurance regarding pre-authorizations, benefits, coverage, exclusions, and the allowed number of visits for physical therapy.
  • It is the patient’s responsibility to know if Peak Performance is in or out of network. Any information we obtain from the patient’s insurance company is an ESTIMATE only and the patient will be responsible for any remaining balances after insurance processes.
  • It is the patient’s responsibility to notify Peak Performance PT, PC in advance of a missed appointment. Appointments missed three times in a row or frequent cancellations may result in discontinuation of further appointments
